If using the “cosmetic forms” on the Poisons Centre’s website the procedure differs depending on which one of the forms is used. |
Cosmetic form 1
Products that comply with a frame formulation and additional information is not required.
This cosmetic form could be sent as it is, by e-mail, cd or dvd.

Cosmetic form 2 and 3
Products that comply with a frame formulation but additional information is required, according to ‘Rules on specific ingredients of concern and/or the pH value’ (Annex I, CPNP User Manual) (form 2) and products that do not comply with a frame formulation or if there is no frame formulation for a given product (form 3).
Create one data file for each product.
If you send information about more than 10 products you also have to create a table in Word-format (see below), which helps us to attach each document file to the right product/trade name and company when it is put into our data base.
In this way our staff quickly finds the right information about a product, when it is needed in the poisons information service. This is very important since it is the product/trade name that is given when calling the Poisons Centre after poisoning incidents. |
How to create the table:
(It is still possible to create a parameter file as earlier, if you are used to that!)
1. Example:
The company Zytx Swe has completed cosmetic forms type 2 and will send them to the Poisons Centre. The name of the manufacturer is Zytx Ltd.
The product names are Zytx Hairspray, Zytx Antidandruff Shampoo and Zytx Hair Bleach. The files are called hairspray.doc, antidan.doc and hairbleach.doc.
The table will look like this: |

N.B. The name of the product and the company will be put into the Poison Centre’s database in exactly the same way as it is written in the table.
The date format must be yyyy-mm-dd!
2. Send the table and product files by e-mail, on cd or dvd to the Poisons Centre.
3. Please, also send a written list with all the product names included in the table. We need this list for administrative reasons.
If you use other forms, in other formats than Words, they will work in most cases, but check with the Poisons Centre to be sure. |
